EVO133 is a potent and highly selective small molecule antagonist of the P2X3 receptor, currently under development by Evotec. The P2X3 receptor is an ATP-gated ion channel primarily expressed on peripheral sensory neurons, where it plays a critical role in mediating pain and the cough reflex. Overactivation of these receptors is associated with the hypersensitization of the cough reflex in conditions such as chronic cough. EVO133 is designed to selectively inhibit P2X3 receptors while minimizing activity against P2X2/3 heteromeric receptors, a strategy intended to reduce the incidence of taste-related side effects (dysgeusia) that have limited the clinical utility of earlier P2X3 antagonists. The drug is currently being evaluated in Phase 1 clinical trials for the treatment of chronic cough and is being explored for other P2X3-mediated conditions, including endometriosis-associated pain.
