Overview

EVO756 is a potent, highly selective oral small molecule antagonist of the mas-related G-protein coupled receptor X2 (MRGPRX2). Developed by Evommune, it is designed as a first-in-class and best-in-class therapy for mast cell-mediated diseases. MRGPRX2 is expressed on mast cells and peripheral sensory neurons; its activation drives non-IgE-mediated mast cell degranulation, leading to histamine release and neurogenic inflammation. By antagonizing MRGPRX2, EVO756 disrupts this pathway, aiming to reduce symptoms such as hives, pruritus (itch), angioedema, and other manifestations of chronic urticaria and inflammatory skin diseases. Clinical studies have shown that once-daily oral dosing of EVO756 significantly inhibits wheal formation in skin challenge tests and provides rapid itch reduction with a favorable safety profile. The drug is currently in Phase 2b trials for chronic spontaneous urticaria (CSU) and Phase 2 trials for chronic inducible urticaria (CIndU), with additional development planned for atopic dermatitis[1][4][6][7].
