Evocalcet is an oral, small-molecule calcimimetic agent developed for the treatment of secondary hyperparathyroidism in patients with chronic kidney disease, particularly those undergoing maintenance dialysis. It acts as a positive allosteric modulator of the calcium-sensing receptor (CaSR) on the parathyroid glands, which increases the receptor's sensitivity to extracellular calcium and subsequently suppresses the secretion of parathyroid hormone (PTH). Evocalcet was designed as an improved derivative of cinacalcet, aiming to provide equivalent efficacy with enhanced bioavailability and a significantly lower incidence of upper gastrointestinal adverse events, such as nausea and vomiting. It was originally developed through a collaboration between Mitsubishi Tanabe Pharma and Kyowa Kirin.
