Overview

Evorpacept + rituximab is a combination therapy under clinical investigation for relapsed or refractory B-cell non-Hodgkin lymphoma (NHL). Evorpacept is a high-affinity CD47–SIRPα fusion protein immune checkpoint inhibitor that promotes tumor cell phagocytosis by blocking the CD47 "don't eat me" signal, enabling macrophage-mediated clearance of malignant cells while minimizing off-tumor cytopenias by employing an inactive Fc domain. Rituximab is a monoclonal antibody targeting CD20 on B cells, facilitating B cell depletion through mechanisms such as antibody-dependent cell-mediated cytotoxicity and complement activation. When combined, evorpacept and rituximab produce synergistic anti-tumor immune activity: evorpacept triggers macrophage engagement for phagocytosis, while rituximab provides an "eat me" signal via its active Fc domain. In clinical studies, this combination has shown a favorable safety profile, with no dose-limiting toxicities and mainly mild-to-moderate adverse events. Promising anti-tumor responses have been reported, with high rates of complete and objective responses, especially in indolent B-NHL[1][3][4][6][7].
