This is a combination therapy comprising **evorpacept** (ALX148, an anti-CD47 fusion protein), **pembrolizumab** (a PD-1 immune checkpoint inhibitor monoclonal antibody), **5-fluorouracil** (a pyrimidine antimetabolite), and **cisplatin** (a platinum-based chemotherapy agent). The regimen is under study for the treatment of advanced or metastatic head and neck squamous cell carcinoma (HNSCC) in patients who have not previously received systemic therapy for their advanced disease. - **Evorpacept**: a high-affinity CD47-blocking fusion protein that promotes antitumor immunity by enabling phagocytosis of cancer cells through the inhibition of the "don't eat me" signal mediated by CD47-SIRPα axis.[5][7] - **Pembrolizumab**: a humanized monoclonal antibody blocking the PD-1 receptor on T cells, thereby enhancing T cell-mediated antitumor response.[4][10] - **5-fluorouracil**: a cytotoxic chemotherapeutic agent that inhibits thymidylate synthase, impairing DNA synthesis and inducing apoptosis in rapidly dividing tumor cells. - **Cisplatin**: a platinum-based compound that forms DNA crosslinks, disrupting DNA replication and transcription, resulting in tumor cell death. This quadruplet regimen is currently in phase 2 trials for HNSCC and being explored for its combined immunomodulatory and cytotoxic effects.[5][9]
