Evorpacept + venetoclax + azacitidine is a combination investigational regimen studied for the treatment of acute myeloid leukemia (AML), particularly in patients unfit for intensive chemotherapy. Evorpacept (ALX148) is a high-affinity, recombinant fusion protein designed as a potent and selective CD47-blocker, acting as an immune checkpoint inhibitor to enhance macrophage-mediated phagocytosis of cancer cells[4]. Venetoclax is a small molecule BCL-2 inhibitor that promotes apoptosis in malignant hematologic cells[3]. Azacitidine is a pyrimidine nucleoside analogue, acting as a DNA methyltransferase inhibitor, resulting in hypomethylation of DNA and cytotoxicity to abnormal hematopoietic cells. The synergy of these three agents aims to enhance anti-leukemic activity by targeting complementary mechanisms: immune evasion, apoptosis resistance, and epigenetic dysregulation.
