Evorpacept + zanidatamab is an investigational combination therapy for cancer that pairs two monoclonal antibodies with complementary mechanisms of action. Evorpacept (ALX148) is a CD47-blocker designed to enhance the immune system’s ability to recognize and destroy tumor cells by inhibiting the “don’t eat me” signal mediated by CD47. Zanidatamab (ZW25; brand name Ziihera) is a bispecific antibody targeting two distinct epitopes on the HER2 receptor, leading to enhanced HER2 signaling blockade and immune-mediated tumor cell killing. The combination aims to improve antitumor activity in patients with HER2-positive or HER2-low advanced cancers—especially metastatic breast cancer—by simultaneously promoting phagocytosis of tumor cells and blocking oncogenic signaling pathways. Early-phase clinical trials have shown promising efficacy and manageable safety in heavily pretreated patients with metastatic breast cancer[1][3][4][5].
