EVT-101 is an experimental, orally available small molecule drug that acts as a selective antagonist of the NMDA receptor subunit 2B (also known as GluN2B or NR2B). It was originally developed by Roche and later advanced by Evotec for the treatment of major depressive disorder, particularly in patients with treatment-resistant depression. EVT-101 binds at the GluN1/GluN2B dimer interface on NMDA receptors, but with a distinct binding mode compared to classic phenylethanolamine antagonists like ifenprodil. The drug has also been investigated for potential use in Alzheimer's disease, Parkinson's disease, and pain (acute or chronic). Clinical trials demonstrated good tolerability and brain penetration at doses predicted to be therapeutically relevant. Development reached Phase II clinical trials for major depressive disorder before being discontinued in 2021[1][3][4][5][6].
