EVX-02B + nivolumab is a combination therapy under clinical investigation for resected malignant melanoma. **EVX-02B** is a DNA-based, patient-specific personalized cancer vaccine that encodes neoantigens—tumor-specific mutated peptides selected by AI-driven algorithms based on the patient’s tumor genomic profile. Each plasmid is custom-designed and manufactured for the recipient. The vaccine aims to induce robust, tumor-specific CD4+ and CD8+ T-cell responses, using the principle that neoantigens elicit a strong immune response with high specificity. **Nivolumab** is a monoclonal antibody immune checkpoint inhibitor (a programmed death-1 (PD-1) receptor antagonist) approved for melanoma and various other cancers. In combination, the rationale is to stimulate and then potentiate anti-tumor T-cell immunity: the vaccine “primes” T cells against the tumor, while nivolumab relieves PD-1 mediated immunosuppression. Early data show that this combination is well-tolerated and associated with durable, neoantigen-specific T cell responses and relapse-free survival in completed patient cohorts.
