Ex-DARP-FGF21 is a long-acting, recombinant fusion protein that functions as a dual agonist of exendin-4 (a glucagon-like peptide 1 receptor agonist) and fibroblast growth factor 21 (FGF21). It is engineered by fusing exendin-4 and FGF21 with an albumin-binding designed ankyrin repeat protein (DARPin) to extend its half-life. The drug targets both the GLP-1 receptor and FGF21 pathways, aiming to improve glycemic control, enhance insulin sensitivity, increase energy expenditure, and promote weight loss. In preclinical studies in mice, Ex-DARP-FGF21 demonstrated significant reductions in blood glucose levels and superior body weight management compared to single-agent controls. Its mechanism leverages the metabolic benefits of both GLP-1 receptor activation (improving insulin secretion and reducing appetite) and FGF21 signaling (enhancing lipid metabolism, increasing insulin sensitivity, reducing hepatic glucose production), making it a promising candidate for treating type 2 diabetes mellitus and obesity[1].
