Ex vivo anergized donor peripheral blood mononuclear cells (PBMC) is an experimental cellular immunotherapy developed by the Dana-Farber Cancer Institute. This biologic therapy is designed for use in patients undergoing haploidentical hematopoietic stem cell transplantation (HSCT) for hematologic malignancies, including acute myeloid leukemia (AML), acute lymphocytic leukemia (ALL), and myelodysplastic syndrome (MDS). The therapy consists of donor T cells that are treated ex vivo through allostimulation in the presence of costimulatory blockade agents, such as CTLA4-Ig (abatacept) or belatacept. This blockade prevents the interaction between B7 molecules (CD80/CD86) on antigen-presenting cells and CD28 on T cells, thereby inducing a state of antigen-specific anergy or hyporesponsiveness in the alloreactive T cell population. The resulting cell product is administered as a delayed infusion post-transplant with the goal of promoting immune reconstitution and providing graft-versus-leukemia (GVL) effects while minimizing the risk of graft-versus-host disease (GVHD).
