Ex-vivo cultured adult allogeneic mesenchymal stem cells (MSCs) are multipotent stromal cells derived from adult tissues and expanded outside the body under controlled laboratory conditions. These MSCs are sourced from healthy donors (allogeneic), not the recipient patient. They possess regenerative properties and can differentiate into various cell types such as bone, cartilage, and fat. Their primary mechanisms of action include immunomodulation—altering immune cell function to reduce inflammation—and secretion of paracrine factors that support tissue repair and regeneration. Ex-vivo expansion allows for large-scale production of these "off-the-shelf" therapeutic products for clinical use. Therapeutically, these MSCs have been investigated in a range of diseases including graft-versus-host disease (GVHD), Crohn's disease, myocardial infarction, spinal cord injury, osteogenesis imperfecta, stroke, multiple sclerosis, diabetes mellitus, leukemia-related complications after hematopoietic stem cell transplantation (HSCT), and cartilage/meniscus injuries[1][2][3]. Their immunomodulatory effects include reducing pro-inflammatory cytokine secretion by dendritic cells and T helper 1 lymphocytes while increasing anti-inflammatory cytokines like IL-10; they also promote regulatory T cell populations[3]. Additionally, they secrete growth factors such as VEGF to support vascular regeneration in damaged tissues[2].
