Ex-vivo expanded allogeneic γδ T cells are an adoptive cell therapy being developed by the Institute of Hematology & Blood Diseases Hospital, China, for the treatment of minimal residual disease (MRD)-positive acute myeloid leukemia (AML) following allogeneic hematopoietic stem cell transplantation (allo-HSCT). This therapy utilizes γδ T cells, a unique subset of T lymphocytes that possess both innate and adaptive immune characteristics, allowing them to recognize and eliminate malignant cells in a non-MHC-restricted manner. The cells are harvested from healthy donors via apheresis, expanded and activated ex-vivo, and then infused into patients to target residual leukemic cells that may survive chemotherapy or the initial transplant. By leveraging receptors such as the γδ T-cell receptor (TCR) and NKG2D, these cells can identify stress-induced ligands and phosphoantigens commonly expressed on leukemic blasts, potentially reducing the risk of relapse without increasing the risk of graft-versus-host disease (GvHD).
