Overview

Ex vivo-expanded cord blood progenitor cells refer to **hematopoietic progenitor cells derived from human umbilical cord blood that have been cultured and expanded outside the body prior to therapeutic use**. The primary objective of ex vivo expansion is to increase the number of progenitor cells, particularly CD34+ hematopoietic stem and progenitor cells, to optimize cell doses for transplantation, especially in adult patients or those requiring higher cell doses than typically available in a single cord blood unit[6][5][1]. The expanded cells are primarily used in hematopoietic stem cell transplantation (HSCT) for treatment of hematological malignancies and other blood disorders[6][5][8]. Expansion is achieved through various techniques, often involving recombinant cytokines (such as stem cell factor, thrombopoietin, and Flt-3 ligand), stromal cell co-cultures, or other supportive agents that promote cell proliferation and/or block differentiation[7][9][1]. While clinical trials indicate that these expanded cells are generally safe and may improve engraftment, challenges remain in retaining stem cell self-renewal and long-term repopulating potential[3][5]. Research is ongoing to optimize expansion protocols and to extend potential applications to regenerative medicine, including neurologic and metabolic diseases[5][6].
