Exegiline (MT-031) is a multifunctional small molecule therapeutic candidate developed by Youdim Pharmaceuticals for the treatment of Alzheimer's disease and other forms of dementia. It is designed as a multi-target directed ligand (MTDL) that incorporates the structural features of the cholinesterase inhibitor rivastigmine and the monoamine oxidase inhibitor rasagiline. Exegiline acts by inhibiting acetylcholinesterase (AChE), butyrylcholinesterase (BuChE), and monoamine oxidase A (MAO-A). By targeting these multiple pathways, the drug aims to enhance cholinergic neurotransmission while simultaneously providing neuroprotective effects through the inhibition of MAO-A, which is involved in oxidative stress and neurodegeneration. This dual mechanism addresses both the symptomatic cognitive decline and the underlying disease progression in Alzheimer's disease.
