This is a combination therapy consisting of three agents—exemestane, tamoxifen, and triptorelin—used in the adjuvant treatment of premenopausal women with hormone receptor-positive early-stage breast cancer. Exemestane is a steroidal aromatase inhibitor that suppresses estrogen synthesis by inhibiting the aromatase enzyme. Tamoxifen is a selective estrogen receptor modulator (SERM) that blocks estrogen receptors on breast cancer cells, preventing their growth. Triptorelin is a gonadotropin-releasing hormone (GnRH) agonist that suppresses ovarian function and thus reduces endogenous estrogen production in premenopausal women. The combination aims to maximize endocrine suppression by both blocking peripheral estrogen action (tamoxifen), reducing systemic estrogen levels (exemestane), and shutting down ovarian function (triptorelin). This regimen has been studied extensively in large phase III trials such as SOFT and TEXT, which demonstrated improved disease-free survival compared to tamoxifen alone or tamoxifen plus ovarian suppression[1][2][3][5][8].
