**exendin-4-IgG4** is a recombinant fusion protein that combines exendin-4, a potent **glucagon-like peptide-1 (GLP-1) receptor agonist**, with the Fc fragment of human IgG4 to extend its half-life. The molecule is designed as a **long-acting GLP-1 receptor agonist** for the treatment of type 2 diabetes mellitus, aiming to improve glycemic control and increase patient compliance by reducing injection frequency. Mechanistically, exendin-4 stimulates insulin secretion in a glucose-dependent manner, suppresses glucagon secretion, inhibits gastric emptying, and promotes beta-cell proliferation and survival. The IgG4 Fc fusion prolongs the drug’s half-life by reducing renal clearance, allowing for extended pharmacologic activity.
