This is a combination of three agents used primarily in research settings: - **Exendin-(9-39)** is a synthetic peptide and a potent, specific, and competitive antagonist of the glucagon-like peptide 1 receptor (GLP-1R). It blocks the insulinotropic and glucagonostatic effects of endogenous or exogenous GLP-1 without agonist activity. Exendin-(9-39) has been studied for its ability to modulate glucose homeostasis and prevent hypoglycemia in conditions such as congenital hyperinsulinism[1][2][3][4][6]. - **Intralipid** is an intravenous fat emulsion composed mainly of soybean oil, egg phospholipids, and glycerol. It serves as a source of parenteral nutrition but is also used experimentally to induce acute lipid elevation or lipotoxicity in metabolic studies. - **Heparin** is an anticoagulant that prevents blood clotting by activating antithrombin III. In research protocols involving Intralipid infusions, heparin may be co-administered to stimulate lipoprotein lipase activity and facilitate triglyceride clearance. This combination does not have an established clinical indication but is frequently used in metabolic research protocols to study insulin secretion dynamics, beta-cell function, lipid metabolism, or GLP-1 physiology.
