EXO 1 is an investigational exosome-based biologic therapy consisting of small extracellular vesicles derived from multipotent mesenchymal stromal cells (MSCs). Developed by the Samara Regional Medical Center Dinasty, it is designed for the treatment of severe COVID-19 pneumonia and associated acute respiratory distress syndrome (ARDS). The therapy is administered via aerosol inhalation, allowing for direct delivery of the exosomes to the lung tissue. EXO 1 functions by modulating the immune response, specifically by regulating the concentration of pro-inflammatory and anti-inflammatory cytokines and reprogramming immune cells toward a regenerative secretome. This mechanism aims to suppress the 'cytokine storm'—an excessive and uncontrolled immune response—and stimulate the repair and regeneration of damaged lung tissue.
