Exofin Fusion is a topical antifungal product developed by Arcutis Biotherapeutics, specifically designed for the treatment of onychomycosis (fungal nail infection). It consists of a terbinafine hydrochloride 1% topical solution paired with a specialized nail debridement tool. Terbinafine, the active pharmaceutical ingredient, is an allylamine antifungal that works by inhibiting squalene epoxidase, a key enzyme in the fungal sterol biosynthesis pathway. This inhibition leads to a deficiency in ergosterol and an intracellular accumulation of squalene, resulting in fungal cell death. The "Fusion" aspect of the product refers to the combination of the pharmacological treatment with a mechanical debridement device intended to improve the penetration of the antifungal solution into the nail bed and enhance clinical outcomes.
