**ExoNMN** is an experimental, cell-free exosome-based therapy consisting of extracellular vesicles isolated from human infrapatellar fat pad mesenchymal stem cells after nicotinamide mononucleotide priming. In preclinical knee osteoarthritis models, intra-articular ExoNMN reduced inflammatory and catabolic signaling, oxidative stress, chondrocyte apoptosis, synovial inflammation, and cartilage damage. Its reported activity is associated with activation of the GAS6-MERTK-PI3K/AKT signaling axis, supporting anti-inflammatory, antioxidant, and cartilage-protective effects. It has been reported in cell and papain-induced osteoarthritis mouse studies and is not an approved therapy.
