Exopolysaccharides from Lactobacillus delbrueckii ssp. bulgaricus OLL1073R-1 (R-1 EPS) are natural immunomodulatory biologic compounds produced by a specific lactic acid bacterial strain. Composed of neutral and acidic polysaccharides (glucose and galactose), R-1 EPS functions as an immunomodulator that enhances host innate immunity by activating natural killer (NK) cells and inducing proinflammatory cytokines. Beyond innate immunity, R-1 EPS has been shown to modulate adaptive immune responses by inducing CCR6+ CD4+ T cells (Th7R) in Peyer's patches, which can enhance the antitumor efficacy of immune checkpoint inhibitors like PD-1/PD-L1 blockades. It is widely utilized as a functional ingredient in yogurt products to reduce the risk of the common cold and is being investigated for its potential to augment immunotherapy in cancer patients, particularly those with non-small cell lung cancer (NSCLC).
