Expanded autologous regulatory T cells (Tregs) are a cell-based immunotherapy being investigated for the treatment of amyotrophic lateral sclerosis (ALS). The therapy involves isolating a patient's own Tregs (typically CD4+CD25+ cells), expanding them ex vivo to increase their number and potentially enhance their suppressive function, and then re-infusing them into the patient. In ALS, neuroinflammation and the loss of Treg-mediated immune suppression are thought to contribute to disease progression. By restoring Treg levels, the therapy aims to modulate the inflammatory environment in the central nervous system and slow clinical decline. This specific program is being studied by the Nanjing Hospital Affiliated to Nanjing University of Traditional Chinese Medicine, often in combination with low-dose interleukin-2 (to support Treg survival) and traditional Chinese medicine (Qu herb).
