Expanded umbilical cord allogeneic fetal mesenchymal stem cells (notably the candidate RESTEM-BD) are a type of cell therapy derived from the Wharton's jelly of the umbilical cord. These cells are expanded in vitro to produce a standardized, off-the-shelf therapeutic product. As allogeneic mesenchymal stem cells (MSCs), they possess potent immunomodulatory and anti-inflammatory properties, mediated through the secretion of various cytokines and growth factors (the secretome). Their mechanism of action involves the suppression of pro-inflammatory T-cell responses and the reduction of systemic inflammatory markers such as TNF-alpha and IL-6, while promoting an anti-inflammatory environment through the release of IL-10 and TGF-beta. This therapy is primarily being investigated for the treatment of severe inflammatory conditions, including Acute Respiratory Distress Syndrome (ARDS) associated with COVID-19, and autoimmune muscle diseases like polymyositis and dermatomyositis.
