ExPEC9V is a 9-valent conjugate vaccine candidate developed for the active immunization and prevention of invasive extraintestinal pathogenic Escherichia coli disease (IED) in adults aged 60 years and older. The vaccine targets nine different O-serotypes of E. coli commonly associated with IED, including bacteremia and sepsis. It works by using sugars (O-polysaccharides) from the bacterial surface, which are conjugated to a carrier protein derived from Pseudomonas aeruginosa exotoxin A via biotechnology processes. This design stimulates the immune system to produce antibodies that recognize and neutralize these E. coli strains without containing live bacteria or causing infection itself. The mechanism of action is not affected by antibiotic resistance mechanisms because resistance is not linked to O-polysaccharide structures[1][2][5]. ExPEC9V is currently being evaluated in Phase 3 clinical trials for efficacy in preventing IED among older adults with a history of urinary tract infections[1][4][6].
