The extracellular vesicle-enriched secretome of cardiovascular progenitor cells (CPCs) is an investigational cell-free regenerative biologic designed for the treatment of ischemic heart disease and heart failure. CPCs naturally secrete a complex mixture of proteins, lipids, and nucleic acids that facilitate cardiac repair; this specific formulation focuses on concentrating extracellular vesicles (EVs), including exosomes and microvesicles, which act as the primary mediators of paracrine signaling. The mechanism of action involves the delivery of bioactive cargo—specifically microRNAs such as miR-210, miR-132, and miR-146a, as well as various growth factors—to damaged myocardial tissue. Once internalised by resident cardiac cells, these molecules activate pro-angiogenic pathways, inhibit cardiomyocyte apoptosis, and modulate the inflammatory response to reduce fibrosis and improve ventricular function. By utilizing a cell-free secretome rather than whole cells, this therapy aims to mitigate risks associated with cell transplantation, such as immunogenicity, while maintaining the therapeutic potency of progenitor cell signaling.
