Extracellular vesicles derived from human induced neural stem cells (hiPSC-NSC-EVs or NSC-EVs) are nano-sized biologic particles secreted by neural stem cells generated from human induced pluripotent stem cells. These EVs contain a cargo of microRNAs and proteins that can be transferred to recipient brain cells such as neurons, microglia, and astrocytes. The mechanism of action is primarily anti-inflammatory and neuroprotective; the EVs modulate neuroinflammation by delivering their molecular constituents into target brain cells in a paracrine fashion. Preclinical studies have shown that intranasal administration of these EVs leads to their incorporation into various brain regions and results in reduced activation of microglia and astrocytes, enhanced hippocampal neurogenesis, preservation of motor function after stroke or injury, and attenuation of neuropathological changes associated with Alzheimer's disease. This therapy is being developed as a non-cellular alternative to direct NSC transplantation for treating neurodegenerative diseases such as Alzheimer's disease and stroke[1][2][3][5][6].
