fampridine

Fampridine is an oral, extended-release small molecule potassium channel blocker used to improve walking in adults with multiple sclerosis who have a walking disability. It blocks voltage-gated potassium channels on demyelinated axons, reducing potassium efflux and helping action potentials propagate to improve motor conduction and walking speed. It is marketed in the EU as Fampyra and is closely related to dalfampridine (an extended-release formulation of 4-aminopyridine approved in the US). Typical dosing is 10 mg twice daily about 12 hours apart, and tablets must be swallowed whole due to seizure risk at higher peak concentrations.
