Fanolesomab is a murine (mouse) IgM monoclonal antibody radiolabeled with technetium-99m (99mTc), formerly marketed under the brand name NeutroSpec. It was developed as a radiodiagnostic agent for scintigraphic imaging to aid in the diagnosis of appendicitis, particularly in patients with equivocal signs and symptoms. The antibody targets the carbohydrate moiety 3-fucosyl-N-acetyllactosamine that defines the cluster of differentiation 15 (CD15) antigen, which is expressed on polymorphonuclear neutrophils (PMNs), eosinophils, and monocytes. By binding to CD15 on activated neutrophils and their debris at sites of infection or inflammation, fanolesomab enables localization using nuclear medicine imaging techniques. Its main clinical application was for detecting acute appendicitis without requiring *in vitro* blood cell labeling procedures. However, due to reports of serious and life-threatening cardiopulmonary adverse events—including deaths—marketing was suspended in December 2005[1][2][4][6].
