**FAP CAR-T** refers to chimeric antigen receptor T cell therapy specifically engineered to target **fibroblast activation protein (FAP)**, a protein highly expressed on cancer-associated fibroblasts (CAFs) within the tumor stroma of many solid tumors. These T cells are genetically modified with a CAR that recognizes FAP, enabling them to specifically bind to and kill FAP-expressing cells. FAP CAR-T is distinct from current FDA-approved CAR-T therapies, as it is primarily being investigated for the treatment of solid tumors rather than hematologic malignancies. The rationale is to eliminate the tumor-promoting fibroblast stroma, which supports tumor growth and immunosuppression, thus enhancing anti-tumor efficacy. Preclinical studies have demonstrated that FAP CAR-T cells can effectively kill both human and murine FAP-positive tumor cells and CAFs, with promising tumor growth inhibition and an acceptable safety profile in animal models, though careful monitoring for on-target off-tumor toxicity (to normal tissues that express FAP) is warranted. FAP CAR-T is currently at the investigational/research stage and not FDA-approved for any disease.[1][5][11][16]
