This combination neoadjuvant therapy regimen consists of the Rho-associated protein kinase (ROCK) inhibitor fasudil hydrochloride, the PD-1 monoclonal antibody toripalimab, and androgen deprivation therapy (ADT). It is currently being investigated in a Phase 2/3 clinical trial for the treatment of patients with locally advanced or oligometastatic prostate cancer prior to radical prostatectomy. Fasudil is intended to modulate the tumor microenvironment and potentially enhance the efficacy of the immune checkpoint inhibitor toripalimab by overcoming immune resistance mechanisms. ADT serves as the standard-of-care hormonal backbone, reducing circulating androgens to inhibit tumor growth. The regimen aims to improve pathological complete response (pCR) rates and reduce minimal residual disease (MRD) in high-risk prostate cancer patients.
