FB-121 is a humanized monoclonal antibody biosimilar of ibalizumab (Trogarzo), currently under development by Fountain Biopharma in collaboration with Oneness Biotech for the treatment of HIV infections. As an ibalizumab-class anti-CD4 monoclonal antibody, it functions as a post-attachment entry inhibitor. FB-121 binds to the CD4 receptor on host T-cells, inducing a conformational change that prevents the HIV-1 virus from interacting with co-receptors (such as CCR5 or CXCR4) after initial attachment has occurred. This mechanism effectively blocks viral entry into the host cell. The drug is primarily designed for heavily treatment-experienced adults with multidrug-resistant HIV-1 infection.
