FD03-IL-1Ra is an experimental bifunctional recombinant fusion protein designed for the treatment of atherosclerosis. It targets two critical pathways in the pathogenesis of the disease: dyslipidemia and chronic inflammation. The molecule is constructed by linking a nanobody that targets Angiopoietin-like 3 (ANGPTL3) with a human interleukin-1 receptor antagonist (IL-1Ra), which is then fused to a mutated human immunoglobulin gamma 1 (IgG1) Fc region to enhance stability and extend its therapeutic half-life. By blocking ANGPTL3, the drug antagonizes the inhibition of lipoprotein lipase (LPL), thereby lowering serum lipid levels. Simultaneously, the IL-1Ra component antagonizes the Interleukin-1 receptor (IL-1R), inhibiting pro-inflammatory signaling (such as IL-6 expression) that drives the progression and instability of atherosclerotic plaques. Preclinical studies in ApoE-/- mouse models have demonstrated that FD03-IL-1Ra can effectively reduce serum lipids, decrease systemic inflammation, and improve the stability of atherosclerotic plaques.
