Febuxostat is a non-purine xanthine oxidase inhibitor used for the long-term management of chronic hyperuricemia in adults with gout who have an inadequate response to or intolerance of allopurinol. It works by selectively and non-competitively inhibiting xanthine oxidase, the enzyme responsible for converting hypoxanthine and xanthine into uric acid, thereby reducing serum uric acid levels. Febuxostat is taken orally and is generally reserved for patients unable to use allopurinol due to efficacy or tolerability issues. The drug was first approved in Europe in 2008 and subsequently by the FDA in 2009. Clinical trials have shown an increased risk of cardiovascular death compared to allopurinol, so its use is typically limited to those who cannot take other therapies[1][3][5][7].
