Overview
Febuxostat and allopurinol are xanthine oxidase inhibitors (XOIs) used as urate-lowering therapies (ULT) for the management of hyperuricemia and gout. Allopurinol, a purine analogue (brand name Zyloprim), and febuxostat, a non-purine selective inhibitor (brand names Uloric, Adenuric), both function by blocking the enzyme xanthine oxidase, thereby reducing the production of uric acid. These agents are the subject of the IMPULsKF trial (NCT03336203), sponsored by Medical Practice Prof. D. Ivanov, which evaluates their impact on kidney function preservation and the prevention of gout onset in patients with chronic kidney disease (CKD) stages 1–4. The trial specifically compares standard versus ultralow serum uric acid (SUA) targets over a 30-month period to establish optimal ULT regimens for renal protection in patients with asymptomatic hyperuricemia or gout.
