FEC-100 is a combination chemotherapy regimen consisting of three cytotoxic agents: fluorouracil (5-FU), epirubicin, and cyclophosphamide. The "100" refers to the dose of epirubicin at 100 mg/m² per cycle. This regimen is primarily used as adjuvant or neoadjuvant therapy for early-stage breast cancer, particularly in patients with high-risk node-negative or node-positive disease. Each cycle typically includes fluorouracil 500 mg/m², epirubicin 100 mg/m², and cyclophosphamide 500 mg/m² administered intravenously every 21 days for up to six cycles[2][5][6]. Epirubicin acts by intercalating DNA strands and inhibiting topoisomerase II; fluorouracil inhibits thymidylate synthase disrupting DNA synthesis; cyclophosphamide alkylates DNA leading to cell death[4][7]. The regimen has demonstrated improved disease-free and overall survival compared to lower-dose anthracycline regimens but carries risks such as myelosuppression, nausea/vomiting, alopecia, mucositis/stomatitis, cardiotoxicity (from epirubicin), and rare secondary leukemias[1][8].
