Fecal filtrate transplant (FFT) is an investigational microbiome-based biological therapy developed by the Academisch Medisch Centrum (Amsterdam University Medical Centers). It consists of a sterile, bacteria-free filtrate derived from the stool of lean, healthy donors, which is highly enriched with bacteriophages—viruses that infect and modulate specific bacterial populations. By transferring the donor's "virome" without the bacterial cells, FFT aims to reshape the recipient's gut microbiota to improve glucose metabolism and insulin sensitivity. It has been primarily studied in clinical trials for the treatment of metabolic syndrome and insulin resistance, representing a refined alternative to traditional fecal microbiota transplantation (FMT) that focuses on the therapeutic potential of the phageome.
