Overview

Fecal microbiota transplantation (FMT) is an investigational microbial therapy being evaluated by the Fondazione Policlinico Universitario Agostino Gemelli IRCCS for the treatment of mild-to-moderate ulcerative colitis. The approach utilizes "superdonor" stool—samples from healthy individuals identified as having high microbial diversity and a history of inducing clinical remission in recipients. By transferring a complex community of commensal bacteria, FMT aims to correct intestinal dysbiosis, restore the production of anti-inflammatory metabolites like short-chain fatty acids, and modulate the host's mucosal immune system to reduce colonic inflammation. In clinical trials conducted by the Fondazione Gemelli, the therapy is typically administered via colonoscopy followed by a series of enemas to ensure adequate colonization of the recipient's gut.
