Fecal Microbiota Transplantation (FMT) is a microbial therapy being evaluated by the University Hospital, Ghent, for the decolonization of Gram-negative multi-drug resistant organisms (MDRO) in hospitalized patients. The therapy involves the transfer of a prepared suspension of fecal matter from a healthy donor (allogenic) or the patient's own pre-colonization sample (autologous) into the gastrointestinal tract. The mechanism of action relies on 'colonization resistance,' where a diverse and healthy microbial community outcompetes resistant pathogens for niches and nutrients, produces inhibitory metabolites (such as short-chain fatty acids and bacteriocins), and modulates the host's mucosal immune system to eliminate the MDROs. This approach aims to provide a treatment option where spontaneous clearance fails, potentially reducing the need for patient isolation and the risk of subsequent infections.
