Fecal microbiota transplantation + vedolizumab is an investigational combination therapy developed by McMaster University for the treatment of active ulcerative colitis. This dual-action approach combines weekly fecal microbiota transplantation (FMT), administered via enema, with vedolizumab induction therapy. FMT is a biologic therapy that aims to restore gut microbiome diversity and address microbiota dysbiosis by transferring intestinal bacteria from healthy anonymous donors to patients. Vedolizumab is a humanized monoclonal antibody that targets the α4β7 integrin, inhibiting leukocyte trafficking into the intestinal mucosa. The combination is specifically being studied in patients who have failed prior biologic therapies to determine if addressing both immune trafficking and microbial manipulation simultaneously leads to superior clinical outcomes.
