**feladilimab + bintrafusp alfa** is an investigational combination regimen comprising two novel immunotherapies: - **feladilimab (GSK3359609)** is a humanized IgG4 monoclonal antibody targeting **Inducible T-cell CO-Stimulator (ICOS)**, acting as an agonist to stimulate T-cell-mediated anti-tumor responses. - **bintrafusp alfa (M7824)** is a bifunctional fusion protein composed of the extracellular domain of transforming growth factor beta (TGF-β) receptor II (acting as a TGF-β “trap”) fused to a human IgG1 antibody against **programmed death-ligand 1 (PD-L1)**. This simultaneously inhibits two key immunosuppressive pathways (PD-L1 and TGF-β) in the tumor microenvironment. The combination is being evaluated in early-phase clinical studies for solid tumors, particularly in the context of advanced cancers refractory to standard therapies. The rationale for this combination is to synergistically enhance anti-tumor immunity by dual checkpoint blockade (PD-L1, TGF-β) and stimulation of ICOS-mediated T-cell activation[3].
