Feladilimab + pembrolizumab + fluorouracil + platinum-based chemotherapy is an experimental combination regimen evaluated for use in advanced or metastatic cancers, primarily head and neck squamous cell carcinoma (HNSCC). Feladilimab is a humanized monoclonal antibody agonist of the inducible T-cell costimulator (ICOS) that stimulates T-cell activation and enhances antitumor immune responses. Pembrolizumab is an immune checkpoint inhibitor targeting programmed cell death protein 1 (PD-1), blocking negative regulation of T-cell function, thus enhancing immune-mediated tumor cell killing. Fluorouracil (5-FU) is a pyrimidine analog antimetabolite that inhibits thymidylate synthase and disrupts DNA and RNA synthesis. Platinum-based chemotherapies (cisplatin or carboplatin) induce DNA crosslinking and apoptosis in rapidly dividing cancer cells. The rationale for this combination is enhanced immunogenic cell death and T-cell activation leading to potentially improved antitumor responses compared to immunotherapy or chemotherapy alone. This specific regimen was studied in randomized controlled clinical trials, but development of feladilimab was discontinued by GSK for this indication due to insufficient benefit in late-stage clinical studies[1][3][5][9].
