Feladilimab + pembrolizumab + fluorouracil + platinum-based chemotherapy is an experimental combination regimen evaluated for use in advanced or metastatic cancers, primarily head and neck squamous cell carcinoma (HNSCC). Feladilimab is a humanized monoclonal antibody agonist of the inducible T-cell costimulator (ICOS) that stimulates T-cell activation and enhances antitumor immune responses. Pembrolizumab is an immune checkpoint inhibitor targeting programmed cell death protein 1 (PD-1), blocking negative regulation of T-cell function, thus enhancing immune-mediated tumor cell killing. Fluorouracil (5-FU) is a pyrimidine analog antimetabolite that inhibits thymidylate synthase and disrupts DNA and RNA synthesis. Platinum-based chemotherapies (cisplatin or carboplatin) induce DNA crosslinking and apoptosis in rapidly dividing cancer cells. The rationale for this combination is enhanced immunogenic cell death and T-cell activation leading to potentially improved antitumor responses compared to immunotherapy or chemotherapy alone.
