Overview

Fenspiride is an oxazolidinone spiro compound with antispasmodic, bronchodilator, and anti-inflammatory properties. It was primarily used for the treatment of acute and chronic inflammatory diseases of the ear, nose, throat (ENT), and respiratory tract—including rhinopharyngitis, laryngitis, tracheobronchitis, otitis, sinusitis—as well as for maintenance therapy in asthma. Fenspiride acts as an alpha-1 adrenergic receptor blocker and a histamine H1 receptor antagonist; it also inhibits phosphodiesterase enzymes PDE3, PDE4, and PDE5. Additional mechanisms include inhibition of neurogenic bronchoconstriction (likely via antimuscarinic action) and suppression of certain inflammatory signaling pathways in macrophages. The drug was withdrawn from markets in Russia and several European countries due to concerns over QT prolongation and risk of torsades de pointes[1][3][6][8].
