Fepixnebart (LY3016859) is a high-affinity humanized immunoglobulin G4 (IgG4) monoclonal antibody developed for the treatment of chronic pain conditions such as diabetic peripheral neuropathic pain and osteoarthritis pain. It functions by binding to key residues in the C-terminal regions of transforming growth factor-α (TGF-α) and epiregulin, thereby preventing their interaction with the epidermal growth factor receptor (EGFR). This selective neutralization aims to modulate EGFR signaling without causing the skin toxicity typically associated with broader EGFR inhibition. Fepixnebart has also been investigated in patients with diabetic nephropathy, where it was well tolerated but did not significantly reduce proteinuria or albuminuria. The drug is being developed by Eli Lilly.
