Overview

Ferumoxides is a superparamagnetic iron oxide (SPIO) contrast agent used in magnetic resonance imaging (MRI) primarily for the detection and evaluation of liver lesions. Developed by Advanced Magnetics, it consists of non-stoichiometric magnetite crystals coated with dextran. Upon intravenous administration, the particles are selectively taken up by the phagocytic Kupffer cells of the reticuloendothelial system (RES) in healthy liver and spleen tissue. The superparamagnetic nature of the iron oxide particles significantly shortens the transverse relaxation time (T2), resulting in a marked decrease in signal intensity (darkening) of normal parenchyma on T2-weighted images. Because most malignant tumors (such as metastases or hepatocellular carcinoma) lack functional Kupffer cells, they do not sequester the agent and appear relatively hyperintense against the darkened background of normal liver tissue, thereby enhancing lesion detectability and characterization.
