FGF23 neutralizing antibody refers to a class of monoclonal antibodies designed to bind and inhibit Fibroblast Growth Factor 23 (FGF23), a bone-derived hormone that regulates systemic phosphate homeostasis and vitamin D metabolism. By neutralizing FGF23, these antibodies prevent its interaction with the FGFR-Klotho receptor complex in the kidney, thereby increasing renal phosphate reabsorption and the production of 1,25-dihydroxyvitamin D. This mechanism is primarily utilized to treat phosphate-wasting disorders such as X-linked hypophosphatemia (XLH) and tumor-induced osteomalacia (TIO). Emerging research, such as that presented at ASH 2025, indicates that FGF23 neutralization may also mitigate bone loss and chronic inflammation in Sickle Cell Disease (SCD) by modulating the TLR4-inflammasome pathway and reducing proinflammatory cytokines like TNFα and NLRP3.
