Overview

FH-FOLR1 CAR T cells are an investigational autologous chimeric antigen receptor (CAR) T-cell therapy developed by the Fred Hutchinson Cancer Center. The therapy involves genetically engineering a patient's own T cells to express a CAR that specifically targets Folate Receptor 1 (FOLR1), a protein overexpressed on certain cancer cells. The CAR construct incorporates an anti-FOLR1 binder derived from farletuzumab, an IgG4 intermediate spacer, and 4-1BB/CD3zeta signaling domains to promote T-cell activation and persistence. FH-FOLR1 CAR T cells are administered intravenously following lymphodepleting chemotherapy, typically with fludarabine and cyclophosphamide. The therapy is currently being evaluated in early-phase clinical trials for pediatric patients with relapsed or refractory acute myeloid leukemia (AML), particularly those with CBFA2T3-GLIS2 fusion-positive infant AML, and is also advancing to trials for relapsed/refractory osteosarcoma at Fred Hutchinson Cancer Center and Seattle Children's Hospital. Preclinical studies have demonstrated potent antitumor activity with complete tumor eradication in osteosarcoma xenograft models.
