Fianlimab is a fully human monoclonal antibody that targets the immune checkpoint receptor lymphocyte activation gene-3 protein (LAG-3, also known as CD223) on T cells. By blocking LAG-3/MHC class II–driven T-cell inhibition, fianlimab acts as an immune checkpoint inhibitor and has potential antineoplastic activity. It is being developed primarily for the treatment of advanced melanoma and other cancers, often in combination with cemiplimab (an anti-PD1 monoclonal antibody), to overcome resistance to PD1 inhibitors and enhance T cell activation against tumors. Fianlimab was invented using Regeneron's VelocImmune technology and is under investigation in multiple clinical trials for various solid tumors[1][2][3][5][6].
