Overview

Filgotinib + methotrexate is a combination therapy used for treating rheumatoid arthritis (RA). This combination consists of filgotinib, a selective JAK1 inhibitor, and methotrexate (MTX), a conventional synthetic disease-modifying antirheumatic drug (csDMARD). ## Efficacy and Clinical Use Filgotinib in combination with MTX has demonstrated significant efficacy in treating patients with rheumatoid arthritis who have had an inadequate response to MTX alone. Clinical trials, particularly the FINCH series (FINCH 1, 2, and 3), have shown that this combination provides rapid and clinically meaningful improvements in disease control[1][3][5]. In the FINCH 1 trial involving Japanese patients with RA and inadequate response to MTX, filgotinib 200 mg once daily combined with MTX showed American College of Rheumatology 20% response rates of 77.5% at Week 12, compared to 36.8% for placebo plus MTX[1]. By Week 24, 65.0% of patients receiving filgotinib 200 mg plus MTX achieved Disease Activity Score with 28 joints (DAS28) <2.6, compared to only 5.3% in the placebo group[1]. The combination has also shown effectiveness in reducing pain. In FINCH 1, a Visual Analog Scale (VAS) pain score reduction to 20 mm or less along with DAS28-CRP remission was achieved by 35.8% of patients receiving 200 mg filgotinib plus MTX, compared to only 11.6% of patients receiving placebo plus MTX[10]. ## Safety Profile Filgotinib combined with MTX has demonstrated a generally favorable safety profile. In Japanese patients from the FINCH 1 trial, the incidence of serious infections was 2.5% with filgotinib 200 mg plus MTX, compared to 5.3% with placebo plus MTX[1]. A pooled safety analysis across the FINCH studies showed that the frequency of major adverse cardiac events (MACE), herpes zoster virus, deep vein thrombosis (DVT), and pulmonary embolism (PE) was similar across treatment groups[4]. The incidences of DVT/PE were less than 0.1% in the filgotinib 200 mg plus MTX treatment groups[4]. ## Mechanism of Action Filgotinib is a JAK1-preferential inhibitor that selectively targets JAK1 over other JAK enzymes. It is nearly 30-fold more selective for JAK1 compared to JAK2[6][8]. JAK1 mediates inflammatory cytokine signaling, while JAK2 and JAK3 are important for hematologic and immune functions[6]. The JAK-STAT pathway is continuously active in RA patients and contributes to disease progression. Filgotinib works by inhibiting JAK1 phosphorylation and preventing STAT activation, which reduces proinflammatory cytokine signaling[6]. This targeted approach may offer advantages over less selective JAK inhibitors by potentially reducing side effects associated with pan-JAK inhibition[8]. Methotrexate, the combination partner, is a conventional synthetic DMARD that has been a cornerstone of RA treatment for decades. In conclusion, filgotinib + methotrexate represents an effective treatment option for patients with moderate to severe rheumatoid arthritis, particularly for those who have had an inadequate response to methotrexate alone.
