G-CLAC is a combination chemotherapy regimen consisting of granulocyte colony-stimulating factor (G-CSF, typically filgrastim), clofarabine, and cytarabine. It is primarily utilized as a salvage therapy for patients with relapsed or refractory acute myeloid leukemia (AML) and other high-grade myeloid neoplasms. The regimen employs G-CSF to 'prime' quiescent leukemia cells into the S-phase of the cell cycle, thereby increasing their susceptibility to the cytotoxic effects of the nucleoside analogs. Clofarabine, a second-generation purine analog, inhibits DNA polymerase and ribonucleotide reductase, while cytarabine, a pyrimidine analog, is incorporated into DNA to terminate chain elongation and inhibit DNA synthesis. This combination is often compared to other intensive salvage regimens like G-CLAM (which includes mitoxantrone) and is studied for its ability to achieve complete remission in patients who have failed standard induction therapies.
